Referrals at HealthPoint Rehab

Healthpoint Rehab accepts referrals from physicians for physical, occupational and speech therapy services.  As a patient you have the right to choose your place of service. The experienced staff, the excellent equipment and facility makes HealthPoint Rehab a natural choice for your services. To set up an initial evaluation with a therapist, please call us in Cape at 573.986.4411 or in Jackson at 573.243.9165.  You may fax your prescriptions to 573.986.4445 in Cape or 573.243.0994 in Jackson or bring them to your first appointment.

Patient should wear comfortable clothing.  For shoulder problems you may want to layer with a sleeveless shirt or we have patient gowns available to use.  If you are being seen for a knee or leg problem, we have shorts available or you can bring your own.

Healthpoint Rehab will file your insurance claims.  Please bring any insurance cards with you to the first appointment.
